A Care Plan is a road map for the resources and services a client requires and includes recommendations for those resources and services. The Care Plan is based on the initial assessment with you and your loved ones and is geared to improving the client’s quality of life. The assessment should be done by a experienced Care Manager.

An Individualized Care Plan can include recommendations to the following:

  • Placement locations
  • Medical providers
  • Durable medical equipment
  • Transition services
  • Homecare services
  • Financial and legal professionals
  • Family support
  • Dementia specialists
  • Support Services
